Based on provider feedback and the need to continue to develop a range of outcome measures we have made the following changes:
- Emergency Ambulance attendance (exception is Domiciliary care)
- Completion of Standard Medication Reviews in Care Homes
- Whether the service uses a health deterioration tool (to be extended to LD residential providers)
- Mandatory Positive Behaviour Support Training has been substituted for Pressure ulcer prevention training as mandatory. (We recognise Positive behaviour training will continue to be highly relevant for some provider and will form a part of their bespoke training offer)
These additions are intended to support an understanding of your participation in, and implementation of, improvement programmes within your services and may act as an additional measurement tool to evaluate the effectiveness of preventative approaches and quality improvement programmes available within Somerset.
